Health Claims - HCB (Hospital Cash Benefit) Rider : In case of Hospitalisation due to Corona Virus, claim is payable as per the product features of HCB Rider. Critical Illness (CI) Claims : Corona Virus is not covered under Criticare+ plan or Critical Illnesses Rider. Hence, the Claim is not payable for diagnosis of Corona Virus Infection. All benefits/claims subject to terms and conditions of the policy.

Your Checklist for Documents Submission to Fulfill A Death Claim

In case of Death Claim: We shall be given a written notice of the Life Insured’s death and, shall be provided with the following documents for us to assess the claim:

Mandatory Documents:

  • ​Claimant Statement Death Form
  • Original policy document (Not necessary in case of dematerialised policy document)
  • Death certificate issued by local authority
  • Claimant's PAN CARD
  • Claimant's address proof
  • Cancelled cheque
  • Claimant's current passport size photograph
 

Additional documents if hospitalisation/ death due to illness:

  • Medical cause of death certificate
  • Medical records for all the treatments taken in the past. (Admission notes, History / Progress sheet, Discharge / Death summary, Test reports, etc.)

Additional documents if hospitalisation/ death due to accident:

  • First Information Report (FIR),
  • Panchnama / Inquest report,
  • Post-mortem report (PMR),
  • Driving license,
  • Police Final Report,
  • Viscera report (if applicable)
  • Newspaper cutting (s), if any,
  • Others as applicable

The claim is required to be intimated to us along with all necessary claim documents required within 90 days from the date of death. However, we may condone the delay in claim intimation, if any, provided valid reasons are given for the delay.

Receipt of the claim intimation given to claimant does not amount to acceptance of claim by the Company under the Policy and is subject to review by the Company. The decision on acceptance and admissibility of the Claim will be communicated separately by the Company to the claimant. The claim is required to be intimated to us along with all necessary claim documents required within 90 days from the date of death. The claim money is paid to the beneficiary who will generally be one of the following:

  • Nominee
  • Assignee in case of assignment..
  • The appointee in case the nominee is a minor.
  • The proposer in case the policy is not on own life.
  • Life Assured himself/ herself in case of policy on own life for living benefit claims i.e. claims under disability, critical illness and hospital cash benefit.

If a claim is not settled within the stipulated time of 15 days from date of intimation for non-investigative cases and 45 days for investigative cases, the Company shall pay penal intertest at a rate, which is 2% above bank rate from the date of intimation till the payment except in the case of claims where an application is made under section 47 of the Act to the Court.
